Enrollment Form

Brothers and Sisters:
We have completed our transition into the new voluntary group disability policy. The new carrier is Guardian. We are fortunate to have an open enrollment for members that are not in the policy currently.

If you would like coverage (or to increase your coverage if you are enrolled in one or the other), print and fill out the attached enrollment form. Short-term is $30.45/mo and long-term is $51.00/mo. The coverage is the same as the Standard Insurance Company offered.

http://www.blet28.org/files/enrollform.pdf

Please return the form to me at:

BLET Division 28
1531 N. Santa Rosa Ave
Tucson, AZ 85712

The enrollment period is brief, so please don’t wait too long. I will be going to El Paso today, and returning, …well your guess is as good as mine.

Please feel free to call if you have trouble printing or with any other questions.

Fraternally,
